I haven't seen factory installed that look that bad, but I may not see as many 'fresh, out of the box' factory rifles either. What I do see is a rapidly rising trend to "do it yourself", and there's nothing wrong with that until it becomes a hack job instead of a good repair.

That was done with a hammer. (The factory uses a press with a mandrel inside the bolt nose) I have seen lots of them that were filed after riveting, but no factory installation that hammered the rivet.
As shortgrass said, just file it square with the rest of the bolt nose ring.
